Please note, I will need a new ENGAGEMENT LETTER from every client BEFORE I can start working on your taxes. Please download it here and either bring it in at the time of your appointment, drop it off with your tax info, fax or email it to me.

If you are a new client, I will need:

1. A copy of each signer’s Driver’s License or other form of state issued ID.
2. A copy of the previous year’s tax return.
3. A signed Engagement Letter. (Below)
4. A Tax Client Info Sheet. (Below)

If you have children, I will need more information.  Please provide a copy of their birth certificate, social security card and a statement and/or bill from their Doctor’s office or their school (or something similar) that shows their name and your address.  I will also need you to fill out my Dependent Credit Questionnaire:  Dependent Credit Questionnaire

If you are a returning client and have moved, please print the Tax Client Info Sheet and fill it out.  And, if you have gotten a new driver’s license, I will need a copy of that, too.
